This 6-week, curriculum-based program builds independent living skills and knowledge for nursing and rehabilitation facility residents interested in transitioning into an independent living setting. Stepping Stones helps individuals improve their communication skills, develop positive peer relationships, and participate in a structured and supportive program.

About the Program

Stepping Stones meets in a 2-hour session, once a week for six weeks. Topics covered include:

  • Independent Living: Disability Rights
  • Social Support: Communication
  • Introduction to Technology: Tablets
  • Housing & Healthcare
  • Income, Transportation, & Money Management
  • Healthy Living
An Access Living instructor teaches nursing home residents about independent living skills.
Access Living staff work to ensure disabled people who want to transition out of a nursing home have the skills to do so.

Costs & Location

Stepping Stones is offered at no cost and facilitated within the nursing facility or rehab center.

The program can be led Monday-Friday between 9:00 am – 4:00 pm. Access Living staff work with nursing facility/rehab center staff to determine the best time to hold the program at that location.

What’s Provided

Facilities hosting the program are asked to:

  • Provide a private space for the group to meet.
  • Identify 8-10 residents who are interested and willing to commit to attending the program

Access Living provides:

  • A group facilitator and interpreter, if needed.
  • Materials for the participants.
  • Snacks for participants that attend the session
  • Participants that complete the program will also receive a $25 gift card and a graduation party.
