Position: Colbert Program Housing Locator

Basic Function:

Access Living seeks a full-time employee to serve as Access Living’s Colbert and Williams program Housing Locator. Each Housing Locator is responsible for implementing programs that achieve and advocate for consumers to obtain integrated housing options when they have a goal to deinstitutionalize from congregate care facilities.

Specific Duties:

  1. To assist consumers to find and move into safe and habitable housing in neighborhoods that meet the desired amenities of the consumers.
    • Locate housing, conduct pre-inspections of units for accessibility and consumer’s safety, and complete required documentation for funding requirements
    • To maintain a list of housing providers that offer adequate and safe housing such as different-sized units in different parts of Cook County
    • Offer each consumer 3-5 safe, sanitary units that meet or can meet (with modification) her/his accessibility requirements
    • Arrange for transportation for consumers and accompany consumers during viewings of offered units and orient her/him to unit’s neighborhood
    • Submit an application for housing subsidies, refer to home modification program, and if needed obtain a reasonable accommodation and secure approval for higher rent payments
    • Support the consumer in getting an executed lease
    • Shop for the consumer for household goods and first month’s food supply, when applicable
  2. To promote integration and independent living philosophy for people with disabilities
  3. To develop working and cordial relationships and conduct regular meetings with DHS, Comprehensive Class Member Transition Program Organizations, and other agencies
  4. Work collaboratively with external multi- disciplinary teams of care coordinators, nursing facility staff, home modification staff, fiscal agencies and other organizations on behalf of Colbert and Williams class members
  5. To learn the housing rights of people with disabilities under Section 504 of the Rehabilitation Act and the Fair Housing Act
  6. Develop partnerships with private owners and providers of subsidized, supportive, and affordable housing
  7. To provide regular weekly updates, complete monthly and quarterly reports, and prepare grant reports when necessary
  8. To properly complete and maintain clients files, including entering data into internal and external online databases in timely fashion
  9. To act as spokesperson for Access Living
  10. To complete other duties as assigned

Education and Training:

Bachelor’s degree preferred. Experience (personal or professional) working with people with disabilities preferred. Education or training in housing inspections desired. Analytical, problem solving, networking, and knowledge of skill-building techniques required. Strong communication skills required.

Experience:

Firsthand knowledge of disability preferred. It will take six months to develop knowledge of accessible integrated housing providers within Cook County, and the system of housing subsidy programs.

Physical Demands: 

Ability to lift up to 50lb. Ability to travel throughout Cook County. Driver’s license and reliable vehicle required.

Access Living requires all employees to have up to date vaccination against COVID-19 unless approved for a medical or religious exemption.

Equipment Operated:

Computers, Fax machine. Employee must have access to reliable transportation to travel throughout the city.

Working Environment:

Up to 70% time might be spent in the community. Office area is a shared workspace divided by cubicles. Verbal communication can be heard from other staff in adjoining cubicles. Access Living operates with a hybrid work plan that allows staff to work in the office, from home, or from the location of their choice (e.g. a coffee shop) each week. Details of this work style will be discussed during interviews for this position.

$39,000 – $44,000
