Deinstitutionalization Program Information

The Deinstitutionalization Program (DI) was pioneered in Illinois by Access Living. Our DI Counselors help people, usually under age 60, who are stuck in nursing homes move into the community. The program helps them find accessible, affordable housing and provides for basic needs to set up a household. DI Counselors evaluate consumers for Personal Assistant Services and other service needs. They provide case management for six months after the client leaves the nursing home until the case is transferred to a Home Services Counselor. Since its inception, the program has liberated more than 200 people from nursing homes so they can live within the community. The process of moving out of an institution often takes a long time, a lot of work, and a lot of patience, but the DI counselors are trained to guide you through the process.
