The Deinstitutionalization Program (DI) was pioneered in Illinois by Access Living. Our DI Counselors help people, usually under age 60, stuck in nursing homes move into the community. The program helps people to find accessible affordable housing and provides for basic household needs to set up a home. The counselors are able to evaluate consumers for Personal Assistant Services and other service needs. They provide case management for six months after leaving the nursing home until the case is transferred to a Home Services Counselor. Since its inception, the program has liberated over 250 people from nursing homes.
