Please be PROMPT... (if you are late you will be asked to leave!) The session will be held at 115 West Chicago Avenue, Chicago, IL 60654.
To participate the day of the session, you MUST
Come in to Access Living, 115 West Chicago Avenue, Chicago, IL 60654 to sign up for the seminar at the front desk, AND
bring the following with you on this day:
- High School Diploma or equivalent
- Two professional written references that we can verify
- A working personal telephone number
- Photo ID and SOCIAL SECURITY CARD copies
- Resume
- Pen
- If applicable, your C.N.A. Certificate
